medical disclaimer

The information provided on this Site is for general informational purposes only and is not intended as medical advice. Nothing on this Site should be construed as an attempt to diagnose, treat, cure, or prevent any disease or condition.

The content on our Site does not establish a provider-patient relationship. You should always consult with a qualified healthcare provider before undergoing any aesthetic treatment or making decisions about your health.

Individual results from treatments vary. Photos, testimonials, and descriptions of treatment outcomes on our Site are not guarantees of specific results. During your consultation, your provider will discuss realistic expectations based on your individual anatomy and goals.

eligibility

Our services are intended for individuals who are at least 18 years of age. By using this Site or booking an appointment, you represent that you are at least 18 years old. We do not knowingly provide services to minors.

appointments & booking

Appointment scheduling is handled through Vagaro, our third-party booking platform. When you book an appointment, you are also subject to Vagaro's terms of service and privacy practices.

Cancellation Policy

We understand that schedules change. If you need to cancel or reschedule your appointment, please provide at least 24 hours' notice. Late cancellations or no-shows may be subject to a cancellation fee. Specific cancellation policies will be communicated at the time of booking.

Consultation Requirement

Certain treatments may require a consultation before scheduling. Our provider reserves the right to decline or modify any treatment based on your individual health history, anatomy, or treatment goals.

Informed Consent

All injectable treatments require signed informed consent prior to treatment. Consent forms are provided digitally through our practice management system and must be completed before your appointment. These forms outline the specific risks, benefits, alternatives, and expected outcomes associated with your treatment. Your provider will review consent with you and answer any questions before proceeding.

communications

By providing your contact information through our Site, booking platform, or intake forms, you may receive communications from us including appointment confirmations, reminders, and follow-up messages through Vagaro.

If you opt in to receive text messages, you consent to receive recurring automated marketing and appointment-related text messages from The Dose Collective at the phone number provided. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for assistance. Your consent to receive text messages is not a condition of purchasing any services.

Treatment-related communications containing protected health information are handled through Quo, our HIPAA-compliant communication platform. See our Privacy Policy for more details on how we handle your information.
